Scheduling Your First Appointment
Bellingham Natural Family Medicine is currently accepting new patients. If you would like to make an appointment, please use the following checklist.
1. Call the clinic at (360)738-7654 during office hours (Monday, Tuesday, Thursday and Friday 9:00am to 12:00pm and 1:00pm to 5:00pm) to schedule an appointment. If you have health insurance, check with your insurance provider to determine the extent of your coverage, or talk with our staff if you have any questions.
2. Click on the links below to download forms for either an adult or a child visit. Print each form and bring completed to your first appointment.
3. Refer to the location page of our website for a map and directions to our clinic.
4. Please arrive 5 minutes prior to your scheduled appointment and bring your completed forms and insurance card.
